Certificate Replacement
If you are already an established patient and lost your International Certificate of Vaccination, you may request a replacement by filling out the secure form below. The Certificate will be mailed to you as soon as possible unless you indicate that you would like to pick it up.
There is a $7 charge for a replacement Certificate whether it's mailed or picked up. After submitting your request, you will be directed to another page where you can make your payment. Please note that this fee is non-refundable. If you are not sure that you have the right travel clinic, please call first.
